This article points in the direction of medical euphemisms used commonly throughout medical codes in hospitals, and for end-of-life patient care. A code replaces words, phrases, or sentences with groups of letters or numbers, while a cipher rearranges letters or uses substitutes to disguise the message. The codes are used to keep a message short or to keep it covert. In colloquial use, the term "code" is often used to mean any method of encryption or concealment of meaning.
