A druggy - it's not that hard to figure out
I am an insomniac who used to have very frequent headaches. This lead me to believe that I was going insane, which scared me to death. Fortunately, however, my migraines have become MUCH less frequent since writing this poem and my insomnia is not a severe as it used to be.
