Sometimes depression becomes all you are. It takes over every thought and every feeling. The pain just sort of becomes you. And then sometimes, you realize that the pain isn't you.
Do you ever have feelings for someone, and you still feel the pain long after they've left? That's the essence of this poem. That torturous feeling was the drive for this poem.
